Request medical records

Patients, parents and legal guardians:

• Submit a signed medical records release form (available in English, Spanish, Creole, and Russian) via fax or by bringing it to the clinic. Healthcare providers can also request for our records to be sent directly to their office with a completed authorization form.​

• Use our ROI vendor, RecordQuest for full medical charts or additional documentation.​ Most requests are processed within 5–10 business days, though older records may take longer.

Third-party requestors:

• All third-party requestors must submit a signed medical records release form (available in English, Spanish, Creole, and Russian). For patients under the age of 18, this form includes the signature of a parent or legal representative. In some cases, additional documents may be required.

• Third-party requestors can use RecordQuest to fulfill their medical record request.

Basic records
Basic records like immunizations, last office visit note, lab results, or school/camp forms may be available for pickup directly at your clinic. Please note, these records can now be made available through your patient portal.

Entire chart

Requests for additional records or the full medical chart must go through RecordQuest. For fee estimates and details please contact RecordQuest. Fees and processing time vary depending on the request type and delivery method (digital vs mailed).
