How to request medical records
Parents and legal guardians
To request your child’s medical records, you may:
Submit a signed authorization form via fax or by bringing it to the clinic. Healthcare providers can also request for our records to be sent directly to their office with a completed authorization form.
Use our ROI vendor, RecordQuest for full medical charts or additional documentation. Most requests are processed within 5–10 business days, though older records may take longer.
Third-party requestors
All third-party requestors must submit an Authorization for Release of Protected Health Information (available in English, Spanish, Creole, and Russian). For patients under the age of 18, this form includes the signature of a parent or legal representative. In some cases, additional documents may be required.

Which records do you need?
Basic records
Basic records like immunizations, last office visit note, lab results, or school/camp forms may be available for pickup directly at your clinic. Please note, these records can now be made available through your patient portal.
Entire chart
Requests for additional records or the full medical chart must go through RecordQuest. For fee estimates and details please contact RecordQuest. Fees and processing time vary depending on the request type and delivery method (digital vs mailed).
Access records via the patient portal
So long, paper files! Your child’s basic records are available to you digitally 24/7 via our secure patient portal. Simply log in to the portal to access these records instantly.

Yes, we can send your child’s records to another healthcare provider with your signed authorization.
Vaccine and immunization records can be accessed and downloaded for free through the patient portal.
School forms can be requested by phone or through the patient's portal. You have the option to wait for an agent or to receive a text message with the link to request forms through the patient portal.
