Whether you’re a new or expectant parent or just new to the area, we welcome you into our extended family. Pediatric Associates accepts new patients in all of our offices.
You will receive a call back from the office to secure your child’s first appointment. If you already have a child registered with our practice, please do not fill out this form.
We want to make your visit as smooth as possible. To save time, please complete the necessary forms ahead of your visit. You can print them out and bring them with you on the day of your appointment. Visit our General Forms and Notices page to get started.

We provide sick care 365 days a year, including evenings, weekends, and holidays. Pediatric Associates offers the most extensive office hours of any private pediatric practice in Florida.
Pediatric Associates also offers 24/7, on-demand pediatric telemedicine services every day of the year through the KidzDocNow app.
Our pediatricians are board-certified/eligible by the American Board of Pediatrics and have trained in some of the best residency programs in the country. Our experienced Certified Pediatric Nurse Practitioners hold Master’s level or higher degrees in nursing and private pediatric practice in Florida.
We want our families to consider us a total resource for their child’s healthcare needs. We are committed to continuity of care and encourage our families to schedule their child’s well visit with the same pediatrician.
All of our offices have separate sick and well waiting areas. Some locations also provide separate newborn waiting areas. Each office has been professionally decorated with warm and welcoming family themes ranging from underwater and space designs to beach, carnival, garden and horse ranch motifs.
