Request Medical Records

Pediatric Associates parents and requesters will use RecordQuest’s Online Request Portal to submit a request for medical records*. Patients and requesters can receive records within a 24-48 hour timeframe from submission. RecordQuest provides bilingual customer service for patients and requesters seeking information on instructions using the request portal, record release status and more. 

*This is exclusively for requesting copies of medical records. For immunization forms, school forms, letters, or any documents requiring a clinician's signature, please contact your clinic directly.

School forms can be requested by phone or through the patient's portal. You have the option to wait for an agent or to receive a text message with the link to request forms through the portal.
