1. WHAT INFORMATION DO WE COLLECT?

We collect personal information that you voluntarily provide to us when you register on our Services, express an interest in obtaining information about us or our Services, when you participate in activities on our Services, or otherwise when you contact us.

Protected Health Information. We may collect Protected Health Information (PHI) when you schedule appointments, access the Company portal, pay bills online, or receive medical care. PHI is handled according to our Notice of Privacy Practices. If we ever combine your personal or usage data with PHI, we will treat all that information as PHI going forward. Information automatically collected. We automatically collect certain information when you visit, use, or navigate our Services. This information does not reveal your specific identity (like your name or contact information) but may include device and usage information, such as your IP address, browser and device characteristics, operating system, language preferences, referring URLs, device name, country, location, information about how and when you use our Services, and other technical information. This information is primarily needed to maintain the security and operation of our Services, and for our internal analytics and reporting purposes. Like many businesses, we also collect information through cookies and similar technologies. The information we collect includes:

  • Log and Usage Data.

Such an offer wall may appear in our application(s) and be displayed to you based on certain data, such as your geographic area or demographic information. When you click on an offer wall, you will be brought to an external website belonging to other persons and will leave our application(s). A unique identifier, such as your user ID, will be shared with the offer wall provider in order to prevent fraud and properly credit your account with the relevant reward.

We do not knowingly solicit data from our market to children under 18 years of age. Access and modify your Personal Information. You may contact us using the information below to request access to or modification of the Personal Information we maintain about you.

Opt-out of marketing and other communications. You can request to manage marketing and certain other communications such as our newsletters and certain other notifications by calling us using the information below or the “Unsubscribe” link in our emails.

Managing Cookies and Other Data Collection Technologies. You have a number of options to manage or limit how we and our vendors use Cookies and other Data Collection Technologies including for advertising:

12. FOR CALIFORNIA RESIDENTS ONLY - WHAT ARE YOUR PRIVACY RIGHTS?

The California Consumer Privacy Act, effective January 1, 2020, provides California residents with additional rights and disclosures. In addition, California’s long-standing “California Shine the Light” law provides that California residents may request certain details about how their personal information is shared with third parties for their direct marketing purposes. The California rights do not apply to Protected Health Information we maintain but you may have other rights in relation to such information. If you are a California resident and have used our Services, you can request a list of the personal information we have shared with third parties for their marketing purposes, if any. We will also give you a list of the third parties that have received your information. You can make a request once each year. In your email, let us know that you are a California resident, who is submitting a California “Shine the Light” inquiry. We will respond to you within 30 days.

We have not sold your personal information in the preceding 12 months. We do, however, reserve the right to share aggregate information and other de-identified information to third parties for their own internal purposes as described in the “When and with whom do we share your personal information?” section of this Website Privacy Statement.

13. INFORMATION ON CONTROLS FOR DO-NOT-TRACK FEATURES

Most web browsers and some mobile operating systems and mobile applications include a Do-Not-Track (“DNT”) feature or setting you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. At this stage no uniform technology standard for recognizing and implementing DNT signals has been finalized. As such, we do not currently respond to DNT browser signals or any other mechanism that automatically communicates your choice not to be tracked online. If a standard for online tracking is adopted that we must follow in the future, we will inform you about that practice in a revised version of this Website Privacy Statement.
